​Physisician's Consent Form

A physician's consent form is only required if:

You have any kind of medical condition that may affect the piercing, the piercing procedure or the ability for your piercing to heal. If you feel you may have a condition that could affect your body's ability to heal, please bear in mind that this may not prevent you from getting the piercing, but we will require that you bring in a physician's consent form which you may download and print out HERE for your physician to fill out. Please bring this in with you when you come in for your piercing. You may also have your physician fill out the form and email it to us at museumoflivingarts@gmail.com. Please make sure the clients full name is in the subject line of the email.

If you are under the age of 14:

In Our body piercing Shops we only pierce earlobes (no cartilage or other body piercings). We do not perform body piercings on children under the age of 14. Earlobe piercings are only done at the age of consent. We do not pierce the earlobes of children who are not old enough to consent to getting a piercing.Earlobe piercings are done on a case by case basis, and requires a consultation with the piercer who is performing the piercing. Whether or not we do the piercing is up to the discretion of the piercer. Because we are regulated by SCDHEC, the following requirements apply. 

The child must have a state or federally issued photo ID such as:  a passport, a military ID, or an identification card issued by the DMV.
✰Unfortunately, school IDs are not acceptable.
The Child must have a parent and/or legal guardian present to sign paper work. The parent or legal guardian  must have a state or federally issued, photo ID.
✰ Either the last name or address must match on the IDs.


If you are 14 or 15 years old:
You must have a state or federally issued photo ID such as: A driver’s permit, a passport, a military ID, or an ID Card issued by the DMV.
✰Unfortunately, school IDs are not acceptable.
You must have your parent and/or legal guardian present with you to sign paperwork. Your parent or legal guardian must have a state or federally issued, photo ID.
✰ Either your last name or address must match on the IDs.

If you are 16 or 17 years old:
Museum of Living Arts age requirements require that you have a state or federally issued photo ID such as: A driver’s permit, a passport, a military ID, or an ID Card issued by the DMV.

*The rules outlined above are state law. Body piercing in SC is regulated by SCDHEC.

The following guidelines are for the health, safety and comfort of our clients and staff. Please take a minute to review these guidelines.


  • Please do not handle your piercings or remove your jewelry in the studio. Our piercer on staff is always happy to assist in the changing of jewelry in the procedure room. This helps to prevent the spread of germs.

  • If you are bringing jewelry in that you are not currently wearing, please bring it in a clean container, such as a plastic bag or tupperware. Do not set loose jewelry down on the counters, glass or other surfaces in the studio.

  • There are several stations throughout the studio for hand sanitizer, as well as hand washing stations. Feel free to make use of them. 

  • If you would like to take photos or videos, please ask the piercer before doing so. 

  • When in the procedure room, please refrain from talking on your cell phone. 

  • Before setting personal items down in the procedure room, ask a staff member where you may set them down so as not to contaminate working surfaces. 

  •  In our industry, like any service based industry, tipping is not expected but accepted. Treat it like you would when visiting a hair salon, nail salon, tattoo artist or massage therapist. Your experience is heavily based on how well the person providing the service does. Tip accordingly. 

  • If the studio is busy and you feel you need more individualized attention then the staff can give you at the moment, please do not rush them. They are working to ensure all of our clients get a top notch experience and will give you the same attention. Let one of our staff know what your time constraints are and they may be able to provide you with a time estimate, or make an appointment for a time that will work better for you. We want to make sure everyone gets the individual attention required for an excellent experience.
